NDIS Support Coordination & Behaviour Support Services
Grounded Support Collective provides clear, consistent support for people navigating the NDIS - bringing the right supports together and ensuring they work in everyday life, especially where things feel complex or difficult to manage. We take the time to understand each person’s situation, build on their strengths, and coordinate supports in a way that is practical, reliable, and aligned with their goals. Our Services

Support Coordination
We support participants to understand and implement their NDIS plans, connect with the right providers, and coordinate supports in line with their goals and preferences.
We specialise in Level 2 and Level 3 support coordination, with experience supporting participants with complex needs - including justice involvement, forensic mental health, and complex psychosocial disability. We navigate fragmented systems, manage multiple providers, and act quickly to stabilise supports and maintain continuity of care.
Our approach is person-centred and strengths-based, focused on building capacity, increasing choice and control, and ensuring supports work effectively in practice. We support participants to navigate complexity, resolve issues, and build confidence to manage supports more independently over time.
-

Behaviour Support
We work with participants, families, and support teams to understand behaviours of concern and the factors contributing to them, and to develop practical, evidence-informed strategies that can be applied in everyday settings.
Our approach is person-centred, trauma-informed, and strengths-based, with a focus on improving quality of life, increasing independence, and reducing the use of restrictive practices. We ensure strategies are clear, consistent, and tailored to each participant’s environment, supporting those around them to respond in ways that are safe, respectful, and effective.

Psychosocial Recovery Coaching
We support participants with psychosocial disability to work towards their recovery goals, build capacity for daily living, and strengthen their ability to manage supports and make informed decisions.
Recovery is not linear. We work alongside participants to build structure, develop sustainable routines, and navigate periods of change or instability. The focus is on practical strategies that support consistency, self-management, and long-term progress at a pace that reflects each person’s circumstances.
